Background: Ipamorelin is a pentapeptide growth hormone secretagogue that acts as a selective agonist of the ghrelin receptor (growth hormone secretagogue receptor, GHS-R1a). By mimicking ghrelin’s action at these receptors in the hypothalamus and pituitary, ipamorelin triggers pulsatile secretion of GH, leading to downstream increases in IGF-1. It was designed to maximize GH release while minimizing off-target hormonal effects. Notably, ipamorelin displays high specificity for GH release: in preclinical studies it robustly stimulated GH but, unlike earlier secretagogues (e.g. GHRP-6 or GHRP-2), it did not significantly elevate levels of ACTH or cortisol even at doses hundreds of times above the GH-effective dose. This selectivity suggests a lower risk of side effects related to stress hormones, making ipamorelin a promising candidate for clinical development in growth disorders or catabolic states.
